Personally, i'd like to see..
Ingame sports: It can be done. Copyrights only apply to huge names and teams,not the actual game itself. It'd also be an interesting diversion from the regular game. Immortals have to do something to kill all of that time..and it would be some repeatable/replayable fun for those that have hit the max cap and basically run out of the usual things to do. (This is usually the biggest issue with any MMO.)
-Baseball
-Hockey
-Wrestling
-Monster vs Monster battles
-Cat racing
-Mount races
-Guild Technology trees
-Guild Siege Wars
-An alignment system to go either good or evil.
-Skill increases that gain experience from use of the skill or weapon.
-More weapon types (Scythes, Bow and Arrow, slingshot, power weapons, fireworks,etc.)
-A "criminal underworld" (If there are going to be wanted posters and a jail, why not an Underworld for the notorious?)
-A Spirit Realm, that would tie in with the alignment system. Good players would lay seige to the "Hellfire" themed areas, while bad players would take to the "Heavenly/Paradise" oriented areas.
-Airship or "Sky Dragon" battles.
